Referee Performance Evaluation Form

The Referee Performance Evaluation Form is designed for coordinators, parents and players to assess game officials effectively. It gathers information such as the referee's name, and date and time of the game being evaluated, among other things. Referees are evaluated in many areas, including appearance, preparation, fitness, character, accuracy of decisions and overall game control. Anyone can complete the form, including players. The form also gathers further comments and feedback.
