Manager Performance Form

The Manager Performance Form is a tool for employees to be able to evaluate their manager. It's a straightforward form which allows the employee to remain anonymous. They can evaluate such areas as communication and motivation skills, and give feedback on which areas the manager excels in or needs to improve.
