Student Registration Form

The Student Registration Form is used by parents to register their K-12 students annually or when starting at a new school. It is a full-scope form which includes basic contact info, emergency and medical info. It also asks for information on the student's former school. The Student Registration Form also has a checklist of documents required for registration.
